You can receive a Health Screening test (for less than 10% of the normal cost) that will provide analysis for over 30 medical factors. This screening is made possible in partnership with Consolidated Laboratory Services, Orange Park Medical Center and by countless hours of volunteer service contributed by members of the Clay County Chamber of Commerce. The panel of tests will screen for the following:
If you participate in a Flexible Spending Account (FSA) and wish to use those funds for the Health Screening, you must pay for the services with a non-FSA card, check or cash and request reimbursement from your plan. To obtain reimbursement, obtain a claim form from your FSA administrator, attach the receipt and a copy of the results and submit to your plan. Also, if your plan covers out of network preventive lab work, you may also submit a claim to your insurance carrier. Obtain a claim form from your carrier, and submit with a copy of the results. The insurance carrier will process it under your plan benefits, which may include deductibles. For plans that reimburse you for cancer screenings (AFLAC, Allstate, Colonial, Liberty National, etc.), males that have the PSA test done may send in a copy of their results to their insurance plan, along with a claim form obtained from the carrier. Many plans reimburse $50 to $75 for the PSA test, regardless of the cost of the test. None of the credit card information that we collect to process your transaction is retained after your transaction is complete. All credit card transactions will be processed within 48 hours of the date of your transaction, no refunds, or credits are given for any transaction.
